Enrollment Requirements
2009/2010
1. Immunization/Medical Records
This information may be obtained from your physician’s office and must be presented within 14 days after student’s first day of attendance, or student will be excluded from school. (Optional use of enclosed form or your physician’s document.)
2. Birth Certification
a. An attested transcript of the birth certificate
b. An attested transcript of a hospital record showing date and place of birth
c. Birth affidavit
d. Passport of attested passport transcript showing the date and place of birth of the student
e. An attested transcript of a baptism certificate of other religious record showing the date and place of birth of
the student
3. Office Forms (contained in enrollment packet)
a. Enrollment Form (1st time enrollee)
b. New Student Profile (1st time enrollee)
c. Pastor’s Recommendation (1st time enrollee)
d. Emergency Information Form (per student)
e. Emergency Medical Authorization cards (x2 per student, for office and teacher)
f. Request For Transcript, if applicable (1st time enrollee)
4. Custody Documentation
A certified copy of order or decree of legal custody must be presented if information was entered on page 2 of the registration form. A letter is also required stating who can pick-up the student from school.
5. Racial/Ethnic Category
White, Non-Hispanic: Persons having origins in any of the original peoples of Europe, North Africa, or the Middle East.
Black, Non-Hispanic: Persons having origins in any of the black racial groups in Africa.
Hispanic: Persons of Mexican, Puerto Rican, Cuban, Central or South American, or other Spanish culture of origin regardless of race.
Asian or Pacific Islander: Persons having origins in any of the original peoples of the Far East, Southeast Asia, Pacific Islands, or the Indian subcontinent. This area includes, for example, China, India, Japan, Korea, the Philippine Islands, and Samoa.
American Indian or Alaskan Native: Persons having origins in any of the original peoples of North America, and who maintain cultural identification through tribal affiliation or community recognition.
Multi Racial: Persons having origins in two or more of the above options.
6. Citizenship Status (Indicate resident status of student on enrollment form.)
a. U.S. citizen
b. Exchange Student – the student has been admitted to the United States under a Department of State
Exchange Program. The student will have a 1AP66 document.
c. Other Non-U.S. Citizen – If student is not a U.S. citizen, notify registrar with copy of registration form and student’s
Immigration Naturalization Service (INS) Number.
7. Foster Care
Parent must have a letter from the agency who placed the student. Send registrar a nonresident form, copy of agency letter, a
copy of the court order.
